Sinus lift – raising the sinus floor when there is not enough bone for implant placement.

Patients often find it very difficult to accept the fact that they do not have enough bone for implant placement and are frequently unnecessarily frightened by this information. In our clinic, sinus lift refers to the elevation of the maxillary sinus floor in order to perform bone augmentation when there is insufficient bone required for proper implant placement in the upper jaw.

In some patients, the sinus floor descends due to previous tooth loss in the upper jaw, while in others this is simply an anatomical characteristic. In such cases, if we want to place implants, we must elevate the sinus floor, meaning we need to perform sinus floor augmentation using artificial bone and a membrane.

Sinus floor elevation or sinus lift can be performed in two ways, depending on the patient’s individual situation:

1. Internal sinus lift – INTERNAL: in cases of smaller vertical bone loss, the sinus floor is elevated during the implant placement procedure itself. THE PATIENT DOES NOT FEEL THIS DURING THE IMPLANT PLACEMENT PROCEDURE. An internal sinus lift can be performed when there is sufficient bone volume (bone width) for implant placement, but 2-3 mm of bone height is missing. After preparing the implant site, the surgeon uses a small instrument to elevate the sinus membrane through the implant site back to its original position and then places artificial bone, which after healing and integration will maintain the sinus at the required height. After elevating the sinus floor and placing artificial bone, the implant is immediately inserted. Most often, after this type of procedure, we wait 4-6 months for proper bone healing and implant integration before proceeding with the fabrication of permanent prosthetic work.
   
2. 2. Classical sinus lift – WITH A SMALL INCISION ABOVE THE TEETH: in cases of greater vertical bone loss, when a large augmentation procedure is required, the sinus is accessed through the outer wall above the roots of the patient’s upper teeth at the level of the sinus floor. Through the so-called lateral approach (“from the side”), the sinus is accessed, elevated to the required height, and larger amounts of artificial bone are added. If the surgeon determines that implants can be placed immediately, this is performed, but healing of the bone and integration of the implants with the bone requires 6-10 months. Only after 6-10 months, depending on the extent of the procedure and determined by the oral surgeon, are the implants uncovered, small metal cylinders – gingiva formers – are placed to shape the gum tissue, and impressions are taken for permanent implant restorations – abutments that will support permanent crowns or bridges made of zirconium porcelain.
